Care Coordination Worker – Integrated Team Care (ITC)

The Care Coordination Worker will work collaboratively to coordinate multidisciplinary care and services for SCMSAC clients with significant chronic health issues. The Care Coordination Worker will operate within the guidelines of the Integrated Care program and will utilise existing infrastructure and partnerships.

The Care Coordination Worker will provide support and assistance to local Aboriginal people accessing health and related services. The Care Coordination Worker will be responsible for identifying barriers to access and in turn, implement solutions to identified barriers, including referrals, advocacy, transport services and follow up support.


  • Provide individual support for Aboriginal people with health related issues, including referral, transport services, advocacy, follow up care and links to other services
  • Liaise with appropriate SCMSAC staff to identify clients with chronic disease who require treatment or follow up care and provide assistance to develop an understanding of their chronic condition and its day-to-day management
  • Assist clients to access services in the region, as well as supporting and enhancing the work of SCMSAC though the provision of care plans and coordination of access to additional, culturally appropriate, safe services required by clients
  • Keep adequate client records through client assessments, contribution to the development of client care plans and linking patients with allied health and specialist services
  • Work closely with clients, carers and primary care teams regarding the coordination and continuity of care
  • Work closely with General Practices in and around the service area in order to promote and enhance access to SCMSAC Services and Programs ITC in particular)
  • Administer the allocation of the Supplementary Services Funding as per the Supplementary Services operating guidelines
  • Support compliance with ITC contract terms, provide accurate and timely communication, including reporting, to Manager/Supervisor and funding bodies as necessary, in order to meet Closing the Gap program requirements
  • Undertake training, participate in staff meetings and evaluation of services as required, in order to ensure quality of service
  • Work closely with Clinical Practices and Committees within the Shoalhaven area to promote access to services at SCMSAC
  • Comply with all SCMSAC Policies and Procedures as well as legal and professional standards including accountability, ethical practice, confidentiality, continuous quality improvement and WHS
  • Other duties as reasonably directed by the Supervisor

Salary will be in accordance with the South Coast Medical Service Aboriginal Corporation Enterprise Agreement 2012 – 2015.
