Chronic Care Support
Our Integrated Team Care (ITC) Program aims to achieve improved health outcomes for Aboriginal and Torres Strait Islander people by assisting patients diagnosed with a chronic condition work together with recommended health professionals and specialists to improve overall health. Our Health Workers can assist you to book your appointment, complete any forms, and liaise with your GP.
We can also help you access Supplementary Services such as:
- Assistance to attend their referred medical specialists appointments
- Assisting with transport costs
- Support with the purchase of medical aids as suggested by specialist
To be eligible for the program, Aboriginal and Torres Strait Islander people of any age should have one or more of the chronic diseases or conditions such as:
- Brain diseases
- Lung diseases
- Kidney diseases
- Heart diseases
- Cancers
